Forms & Sign-Up Information

IMPORTANT:
**A current physical form is required for each student participating in athletics. The form needs to be signed by the physician, parent/guardian, and student-athlete.  When possible please be sure the clinic stamp is on the form. 
Student-athletes will be not be permitted to participate without a current physical on file in the athletic department.**

NOTE:  ATHLETIC REGISTRATION IS NOW ONLINE.  GO TO THE PARENT PORTAL IN INFINITE CAMPUS.
Registering your son/daughter for a sport in Infinite Campus is also agreeing to the Athletic Rules and Regulations as well as the Concussion & Cardiac Arrest Agreement, for both the student-athlete and the parent/guardian.

TEAM MANAGERS:  Team Managers should NOT register online, they will need to turn in the following to the athletic dept.:
1. physical / participation form (one each year)
2. concussion/cardiac arrest form (one each year)

  You can also drop off the physical at the East or West high school athletic departments, or mail to:

• ALTERNATE YEAR:  If your child is on the list of current physicals, you will only need to complete the registration for your sport in Infinite Campus, you will not have to submit any form for the alternate year.
• If your physician gives you a WIAA physical form, that is fine, we also accept those forms.
• We will NOT accept a print out of an office visit summary from My Chart, or from an office visit.  It must be a physical form signed, stamped and dated by the physician.

**Physicals dated after April 1 of the current year are good for the current school year and the following school year. 

Walk-In Physicals:
If you are unable to get an appointment for a physical, you may want to consider "Concentra" in Madison.  They offer walk-in physicals.  Here is the information:

Physicals dated after 4/1/25 will be good for the 26-27 school year. 
Physicals dated after 4/1/26 will be good for the 26-27 and 27-28 school years.

26-27 SPORTS FEE AND REGISTRATION INFORMATION

Athletic Fees:
Fees for each sport will be paid at the time of online registration. If a team member is cut due to tryouts, the
fee will be refunded. Fees are not refunded if a student quits after the first two weeks of practice.
The athletic fee for each sport is $100.00.

Refunds:  Within the first two weeks of practice the fee can be refunded.  Fees are not refunded if a student quits the team beyond the two week point.

Free and Reduced Information:

Students that qualify for the district free and reduced lunch program may be eligible to have their athletic fee
waived. Families need to apply for the free and reduced lunch program EACH YEAR with the School
Nutrition Department. If you know your student will be participating in athletics, and you qualify for this
program, please be sure you have registered with the Sun Prairie Area School District Nutrition Department
before the start date of your child’s sport(s) each year so your child is able to begin their sport without delay.

Online Registration Information

You will log in to your Infinite Campus Parent Portal account to start the process.

Please register before the start date as all registrations are processed before tryouts start. We may not accept registrations at tryouts or after tryouts.

To check to see what sport you have signed up for:

1. go to the school store
2. on the right, click on “My Accounts”
3. go to payment history
4. click on the search range and you should see what you have signed up for by clicking on “completed”.

We do not accept printed registration forms. All registration forms are signed online through the Infinite Campus sign up.
