Forms & Sign-Up Information

IMPORTANT:
**A current physical form is required for each student participating in athletics. The form needs to be signed by the physician, have the date of exam, and clearance for play. When possible please also include the clinic stamp on the form. 
Student-athletes will be not be permitted to participate without a current physical on file in the athletic department.**

NOTE:  ATHLETIC REGISTRATION IS ONLINE.  GO TO THE PARENT PORTAL IN INFINITE CAMPUS.
Registering your child for a sport in Infinite Campus is also agreeing to the Athletic Rules and Regulations as well as the Concussion and Cardiac Arrest Agreement, for both the student-athlete and the parent/guardian.

TEAM MANAGERS:  Team Managers should register online in the Infinite Campus School Store under the team manager option. Parent permission is required to become a team manager.

• If your physician gives you a WIAA physical form, that is fine, we also accept those forms.
• We will NOT accept a print out of an office visit summary from My Chart, or from an office visit.  It must be a physical form signed, stamped and dated by the physician. Electronically signed physicals are accepted, if the physician's signature is dated. 

A few times a year, the Sun Prairie District Wellness Clinic offers free sports physicals for student-athletes that qualify for fee waivers (free/reduced lunch). Reach out to the Sun Prairie East athletic office or watch for social media updates for the next upcoming date. 

Walk-In Physicals:
If you are unable to get an appointment for a physical, you may want to consider "Concentra", in Madison.  They offer walk-in physicals.

Physicals dated before 4/1/25 are expired and the student-athlete cannot participate.
Physicals dated after 4/1/25 will be good for the 25-26 school year.
Physicals dated after 4/1/26 will be good for the 26-27 and 27-28 school years.

Fees:
All Athletic Fees are $100 per sport. Fees for each sport will be paid at the time of online registration.
If a team member is cut due to tryouts, the fee will be refunded.

Note:  If a student-athlete is placed on a roster as a team member, is riding the bus, participating in team events regardless of practice or play, the fee is required.  Team managers are not required to pay the fee.

Refunds:  Within the first two weeks of practice the fee can be refunded.  Fees are not refunded if a student quits the team beyond the two week point.

School Nutrition Dept.:  608.834.6544
Students who qualify for the district’s free and reduced lunch program may be eligible to have their athletic fees waived. Families must apply for the free and reduced lunch program EACH YEAR through the School Nutrition Department. If your student plans to participate in athletics and your family qualifies for this program, please make sure you have completed both the free and reduced lunch application AND the Fee Waiver Form with the Nutrition Department before the start date of your child’s sport each year. Completing these steps ahead of time will help ensure your child can begin participation without delay.
