Our district health plans cover annual vision exams, so if you are covered under our health insurance, you can schedule your annual vision exam directly with SSM Health or any other optometry provider that accepts Dean Health Plan using your health insurance card/information.  

Our vision insurance is a materials-only plan which covers up to $200 through a single allowance towards glasses or contacts.  This plan is offered through Delta Vision using the EyeMed Network.  Here is where you can go to find providers for Vision Hardware.

Eligible Employee:

Any employee hired to work at least 30 hours per week and NOT currently enrolled in a High Deductible Health Plan (HDHP) with a Health Savings Account (HSA) is eligible to participate in the flexible spending plan.

This is a voluntary benefit, which allows you to fund certain specific expenses with Pretax dollars rather than After-tax dollars. Employee Benefits Corporation (EBC) is a Third Party Benefit Administrator providing administration of The BESTflex Plan, EBC's Flexible Spending Accounts (FSA). The BESTflex Plan is a Section 125 Cafeteria Plan governed by the Internal Revenue Service (IRS) that allows certain health care and dependent care expenses to be paid with pretax dollars.

Employees are initially eligible for this benefit the first of the following month of their first date of employment and annually during annual open enrollment thereafter.

Basic Life Insurance:
Sun Prairie Area School District will purchase a term life insurance policy on your behalf equal to 1 x annual salary* rounded to the next higher $1,000; maximum $100,000.There is no cost to you for this coverage. (Taxes will be applied to employee if the coverage exceeds $50,000.)

Optional Life Insurance:
You have the opportunity to purchase additional term life insurance equal to the amount purchased by the District at a cost to you of $.155 per thousand dollars of coverage per month; maximum $100,000.

The amount of Basic and Optional Life Insurance reduces 35% upon your attainment of age 65 and terminates upon your retirement.

*Administration and Administration Support will be covered at 4x salary.

Percent of Covered Salary Insured:
67% of monthly salary for Support Staff/Admin/Admin Support and 90% for Professional Educators

Elimination Period:
60 days for Support Staff/Admin/Admin Support and 60 days for Professional Educators

The Sun Prairie Area School District and the City of Sun Prairie are pleased to offer an employee wellness clinic to all regular part-time/full-time employees, their spouses, and their dependent children (ages 2+). Employees do not have to be a part of the District’s/City’s health insurance plan in order to receive services at the clinic. Seasonal, temporary, and substitute employees; and elected officials are not considered regular part-time/full-time employees. All employee are eligible for on-the-job injuries (worker’s compensation).

Please remember to always report immediately!  Don’t wait over the weekend / next shift to see if symptoms improve before reporting.  You should always notify your supervisor and/or health assistant within 12 hours of your injury, get a yellow Work Comp Claim Kit envelope from your health assistant or admin assistant and then call our TeleComp Care Nurse Triage Line at 1-866-323-4227.  You must provide our Policy Holder Name: Sun Prairie Area School District AND our TCC# 100291.  If you end up needing in-person care, our Employee Wellness Clinic is a great resource (though you always have the choice of where to seek care).  When using the Wellness Clinic, call as soon as possible to ensure they can get you in to be seen as timely as possible.  It is very difficult for them to see you same day if you call at the end of your shift.  Thank you for continuously working with your buildings and teams to identify ways to prevent workplace injuries in the first place, and working diligently to report and recover when unavoidable incidents do occur.
