ATHLETIC REQUIREMENTS FOR STUDENTS PARTICIPATING IN CARDINAL HEIGHTS UPPER MIDDLE SCHOOL SPORTS

If your student is interested in participating in athletics at Cardinal Heights, they will need the following:

  1. The Participation & Physical Agreement Form    
  2. Concussion Acknowledgement Form
  3. Athletic fee: A one time $26 athletic fee which is good for all sports during the year. (8th grade only, 9th grade should see 9th-grade sports). The Athletic fee must be paid online.

Wrestling Practice begins October 21st from 3:30pm-5:00pm at Token Springs Elementary school

Boys basketball practice will begin on Wednesday, October 23rd.

8th Grade Sports Offered at Cardinal Heights

Welcome to Cardinal Heights Upper Middle School.  We are excited to have you  join our school community.  One way to get connected at Cardinal Heights is to participate in 8th grade Athletics.  We offer Volleyball, Cross Country, Wrestling, Boys and Girls Basketball and Track and Field.  We have a place for you whether you have played sports for a long time or just want to give it a try.

Activity Bus

An activity bus is available everyday at 4:10 pm at the front of the Cardinal Heights.
Please note:
Only bus students may ride this bus.  You will need a bus pass from a teacher or coach everyday in order to board the activity bus.

9th Grade Sports Information

Physicals dated before 4/1/17 have expired.
Physicals dated on or after 4/1/17 will be good for the 18-19 school year. 
Physicals dated on or after 4/1/18 will be good for the 18-19 and 19-20 school years.

If you get a new physical during the year, remember to take a physical/participation form with you for your doctor to sign and stamp.  Remember to turn the new physical form in to the athletic department office to keep your records up to date.  The clinics do not automatically send us this information.

What Do I Need Each Season?
For the first sport you do each year, you will need:
1.  Athletic Participation and Physical Form

4.  Athletic Fee (Note:  paid online in Infinite Campus after rosters are complete)

For each additional sport during the same school year, you will need:

Athletic Locks:
Some sports will require an athletic (red dial) lock when students use the locker room. All student-athletes that use the locker room are required to have this school issued lock. The lock is $5.00 and is available for purchase in the athletic office of the high school. Coaches will let you know if a lock is needed for their team. For security reasons, students may not use a personal lock. 

Athletic Fees - Refunds:
Fees are only added to students that make a team.  Refunds are only considered the first two weeks after practice begins.  Students that quit the team after the first two weeks are not eligible for a refund.  NOTE:  Refunds are only good for the current school year.

 

Voluntary Student Insurance Plan

Sun Prairie School District has made available student accident insurance coverage for your child on a voluntary basis through Markel Insurance Company. Parents/guardians can choose the type of coverage that best fits their student's needs; School Time Coverage, Around the Clock Coverage, and Interscholastic Football Coverage.

Reasons to Purchase this Coverage:   

  • High deductible and co-pays in your health plan. Many health plans have increased the amount of out-of-pocket expenses.
  • No insurance. If you have no other insurance, this can become your primary accident plan.
  • This plan will provide benefits for medical expenses incurred because of an accident.

WIAA Head Strong Concussion Insurance

The HeadStrong Concussion Insurance Program was specifically developed to insure student athletes from the high cost of concussion treatment and neurological follow up.

The insurance only covers athletes in WIAA recognized sports.  It does not cover cheer, dance, lacrosse, club sports, or sports students are involved with on their own in their community.
